my pain specialist said, it is uncommon to him, that patients have this kind of pain on both sides. It started with me on the left side, and pain started on the right side one year later.
There was no incidence at all, the pain grew slowly.

Does someone know if this bilaterality is a sign for a particular cause of PN?

What happened to me was the pain started on one side but due to pain the muscles throughout the pelvis went into spasm and I had pain all through the pelvis. But, I don't know for sure the answer to your question in your case.

Several doctors have told me that bilateral PN pain can be an indicator of pain originating at the spine. This is clearly not always the case, and may be less likely as your pain began on one side and spread, but Tarlov Cysts and tethered spinal cord, as well as other issues, can have pudendal nerve pain as a symptom. You can ask your doctor for a lumbar and sacral MRI and get a couple of opinions on the images, maybe one from a knowledgeable neurosurgeon.
