Ilioinguinal and genitofemoral - Dr. Hibner

Ilioinguinal - The radiologist that Dr. Hibner works with is now doing alcohol deinnervation of the ilioinguinal nerve. They have had good success with this.

Genitofemoral - Dr. Hibner is now doing genitofemoral neurectomies by a laprascopic approach. He was going to talk with the radiologist to see if the radiologist could do an alcohol deinnervation of the genitofemoral nerve for those who don't want surgery. He will let me know.

I haven't come across this alcohol deinnervation nyt, interesting. I did google it and came up with this http://www.dr-salim.com/english/pdf/facets_syndrome.pdf but I couldn't find a date on it.
This gives people more options at least, especially if they have had good results. So this is not a diagnostic 'injection'; Is it an injection?? it is offered as a therapeutic option instead of surgery?
I read a bit about the neurectomy you mention it mentioned that the patient may have numbness as an after effect, that would suit me down to the ground with regard to my PCFN obviously the function of the nerve need to be realised

This is NOT a diagnostic injection. Obviously one would need a diagnostic injection first. Alcohol injections to kill a nerve use to be used along time ago and then things like neurectomies and cryoablation came along to replace it but was not an uncommon practice at one time. One would be numb in the area of innervation but some individuals in the published literature I have read report that with time the numbness resolves but that does not happen with everyone. All I know I would prefer numbness to pain.

I read somewhere (I lost the link) that the problem may be worse when the destroyed nerve regenerates and grows again. The pain can be more severe afterward.

Yes agreed nyt, numbness would always be my choice :) . To a small extent I do wonder if some of my extensive numbness would be replaced by pain if or when I do have surgery (still no further forward with that however)
I would imagine that if what ezer mentions does happen, there could be a repeat of the alcohol injections??

I do know with cryoablations that it is not uncommon for the nerve to grow back. I have not read in the literature that the majority of individuals have increase in pain but their pain does come back. I don't know the percentages of how many individuals with cryoablations have nerve regrowth. That is the one advantage of the neurectomy is that unless you end up with a neuroma on the end of the cut nerve that is buried you don't have to worry about regrowth of the nerve. I do not know if the alcohol injections are more permanent than cryoablations as far as regeneration of the nerve.
