I would like to know if all the information for how the prescription should be written is still correct as written at the "Doctors" part of this site. I will be traveling quite a distance for it so I want to get it right:

Hollis G. Potter, MD

The script should be as follows.
MRI of the Pelvis
Bilateral Pudendal nerve
1) From ischial spine to Sacro Spinal ligament beneath Sacrotuberous ligament and to the Alcock's canal.
2) Dorsal Nerve of the clitoris or penis especially passing through the pubic bone.
To be reviewed by Dr. Hollis Potter.

Chief, Magnetic Resonance Imaging
Professor of Radiology
Weill Medical College of Cornell University

The address is as follows:
Diagnostic Radiology
Hospital for Special Surgery
Magnetic Resonance Imaging
535 East 70th Street
New York NY 10021

I suggest you call and verify with Dr. Potter's office. Have you had pelvic imaging done near your home?

When I call the number above I just get dumped into a massive email system where I eventually have to choose "MRI".

Does anyone have a more targeted phone number for Dr. Potter?

I would think that if the prescription is not correct they would not let you schedule your appointment as they would need to know what they were planning to evaluate prior to taking the images.

Does anyone know if the prescription written as in my first post in this thread includes a lumbo-sacral MRI?

It doesn't sound like it. It says MRI of pelvis. I think you would also want an MRI of the lumbosacral plexus as well as a lumbosacral MRI. That is what I had.

Does this script involve looking around the tailbone, rectum, and colon? Would it pickup a rectocele?

I think a rectocele is diagnosed by vaginal examination. http://www.medscape.com/viewarticle/722323_5 I don't know if an MRI would be able to diagnose it, especially if you are lying down.
