Long story,Hello i am 32 years old male having mild lumbar stenosis and a disc protusion at l4 l5 which never gave me Much of a problem ,in august 2014 i had relapse but i recovered soon from pain but it left me with a problem of tingling in scrotum when sitting or lying on my hips and other paraesthesia in groin region and hypersensitivity inside the bums it was all bearable until december when i started havin pain in my right leg till the ankle sciatic nerve which is almost unbearable when i m trying to sit but once i sit i have no pain similarly its painful in many ways when i change postures like changing side when lying down,trying to stand or bend i m always in unbearable pain in my right leg and it travels to the groin at times and front of the thigh apart from this now i always have continous dizziness developed a cataract in one eye for unknown reason for which i had a brain mri done which came out normal ,a nervous stomach cant eat comfortably ,the main thing is drs feel that the sciatic pain is not due to pinched nerve at l4 l5 but somewhere else but not able to diagnose it ad piriformis muscle is okie there is small si joint ussue but drs say s i joint dysfuntion is never so painful ,cant cough sneeze or anything i have 3 kids my whole family is financially dependent on me, can this be pudendal neuralgia i need support ,please help as drs here are not aware of this

Hi Karan,
Sorry to hear you are experiencing so much pain. Often with pudendal neuralgia there is more than just one thing going on. The pudendal nerve doesn't innervate the leg so while you might have irritation of the pudendal nerve causing the pain/symptoms in the saddle area, that would not account for the pain in your leg. Sometimes with PN, muscles in the pelvis go into spasm and cause pain in other places but in my experience the sciatica symptoms that I had were caused by the piriformis muscle being in spasm and impinging on the sciatic nerve. It sounds like you had that ruled out though. Since you have wide-spread pain you have to be suspicious of a radiculopathy in the spine but it can be very difficult to diagnose. Sometimes it is a matter of continuing to search for a doctor who can help you, or trying alternative therapies. Do you have any good physical therapists there who can help you sort this out?

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

Thanks for replying violet why pudendal neuralgia came to my mind was due to tingling in the scrotum area and burning and altered sensations in the skin outside anus ,sometimes dull ache as well and unknown cause of spasms which give me unbearable pain in my right leg ,so came here because u folks r the best people to tell since u have undergone the treatment and this problem so u know and understand the symptoms like noone else ,please can u tell me the primary and early symptoms of pn in men

I m consulting pt but they r not experienced with the pn symptoms they have only read about it but havent ever come across a case and they r also confused about the pins and needle sensation i get in my scrotum and temperature alterations in the surrounding areas as this is not disc related i have been on pregabalin and amytriptyline for a while just tapered it and came back to gabapentin as it was not helping in my parasthesia issue i sometimes get a feeling of a vibration or restricted blood flow to the area mentioned above as well dunno why really feel uncomfortable and the pain in the right leg is icing on the cake

Karan, your symptoms sound typical for what I have heard men describe for pudendal neuralgia although the severe pain in your right leg is not typical. I had some pain in my leg but not severe. It was more of a dull ache with numbness that was caused by the piriformis muscles being in spasm and pressing on the sciatic nerve. The pudendal nerve innervates the skin of the scrotum and innervates the anal sphincter so pain in those areas (including burning pain or pins and needles sensation) is very common with pudendal neuralgia. The early symptoms may vary from person to person, depending on which branch of the pudendal nerve is affected and which nerve fibers are affected. Thanks for bieng a support violet but i only have pins and needles and burning feeling not burning pain it doesnt give me any pain ,just pins and needles intermittently and temperature variations ,thats the only reason i m so confused ,and dont surf much through the site as when i read other members agonies i get a anxiety attack which take hours to get in control as there r no drs for pn here even the best of drs who r worldwide known for thier spine acumen refuse to comment and tell me its my l4 l5 disc problem which gives this symptoms ,regards

Karan, I'm sorry, I thought you said you had burning around the anus. It's very difficult when your doctors aren't able to help you. Try to stay calm and focused on getting treatment. You could try the mindbody approach that Ezer posted about. Maybe that will help you to keep from becoming anxious.

So violet u still feel i have classic features of pne or pn and do u get any dizziness or lightheadedness because of pn,thank u so much for not losing patience and replying it means a lot to me

Karan, I did not get dizziness or lightheadedness with PN. Symptoms of PN can vary from person to person because there are 3 branches of the nerve and other diseases with similar symptoms as PN, so it's hard to say what is classic. They did my 3t conrast mri for brain and it came out negative
My clear issues or symptoms are (my aplologies for bad words or bad explanation)
1 Painless burning feeling in the anus area
2 Tingling in the scrotum which last for 20 seconds and then goes and return after sometime and it keeps happening again and again
3 right leg sciatic nerve pain which is unbearable and referring to inside of thighs and groin
4 and slight pain inside the left cheek
