bilateral nerve block question

Post by rea »

Tomorrow makes 2 wks since had bilateral perineal nerve block from Hibner. I've started severe itching, some swelling, and burning. It's very painful. Pain is gone in perineum right now but my clitorial area is horrible. Sorry, no other way to say that. Only ice helps.
Haven't found out if he did vein screlazation but I see veins, arteries, or the nerve itself are very noticeable. Could this be the steroid causing a reaction? I have a trip to see a doctor that's a 7 hr. drive (Lying down in back) and I'm hoping this goes down before then.

I never want another block but this is the second if they don't count the vaginal block I had with my ob/gyn!

Rea,

Sorry you are having these new problems. I think you really deserve to know if you had the vein scleritization and just to be on the safe side it would not be a bad idea to call Hibner's office tomorrow and let them know what's going on. Some people do have a flare-up after nerve blocks due to the steroids. I had a bad flare-up after one of my blocks -- the one into alcock's. Hopefully yours will go away quickly. Do you have some extra pain meds to get you by?

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

Violet,
Pain medicine is not touching the pain. I am using something for the itching which does help but no relief even with ice, the pain is still there. Also, I can feel and see there is a bulge/swollen lump in my perinium right below the vulva on the left side. I can only feel it when sitting on commode. I've felt a small lump in my left groin but this is not the same location. Just trying to wait and see what happens. I'll try calling Hibner's again. Maybe it's a flare but it's nothing I've felt before. There is short term burning and the itch so something's different from the block.
Thanks Violet.

Rea, I can't tell you what the lump is but since it occurred after the injection I'm thinking of some possibilities -- like a hematoma, a collection of fluid from the injection, an inflammatory response, or some scar tissue formation. If it's one of the first 3 maybe some alternating hot/cold sitz baths (while on a donut cushion) will help. I did those after my surgery and they were very soothing, plus the alternating heat/cold brings fresh blood to the area and promotes healing.

Rea, I was just thinking if you are unable to sit on a donut in the water you could try alternating with a moist electric heating pad and an ice pack.

I've been on a roller coaster with this block. The veins on the left side are still prominent and my pain is worse on left from touch. Hypersensitive is an understatement and pain medicine doesn't help. I do believe the pain is from the nerve but don't have a clue why my symptoms have changed. I don't have pain in perineum now. Have felt very short term burning. My pain does get worse as they day goes on and by the evening I'm miserable so it's the nerve and not a flare. The pain is like none I've ever felt and it is worse on the left side now. Before it was equal bilaterally. It is a sore pain, if that makes sense. Sometimes get the itching. I feel swollen and have to walk bow legged. The knife-like pain is hitting more often. The pain is from the bottom of the mon pubis to the bottom of the vulva (labia majora and minora). It is worse around the clitoris now when before the burning was minimal there (thanks to medicine?).

Loretta said perineal nerves and then Dr. Hibner threw in the dorsal nerve. I think because of genital arousal. Haven't had that since Jan. My MRI found no issue with dorsal nerve.
I know there are risk with nerve blocks and looks like I'm dealing with becoming worse. Is this permanent? Why has the type of pain changed? Is a perineal nerve block done in the clitoris as mine was?

If my dorsal is a problem than I have to see Hibner... but I feel it would be a lost cause and too costly to have surgery with him now. I would like to see Dr. Conway (he's in my network now). I know he doesn't deal with the dorsal but maybe he will learn the technique in the future. Hibner had only done 4 perineal/dorsal surgeries when I saw him in June. I guess I want another opinion.
Thanks so much
