Hip Dysplasia & Infertility with Pudendal Neuralgia

Hello everyone,

I first tried the German forum, but I’ve noticed that there are some people with hip issues in the English forum, so I’m posting here as well:

After 1.5 years of seeing various doctors, I’ve been diagnosed with Pudendal Neuralgia. Since I’ve never had surgery, fractures, etc., I don’t believe the nerve is injured. Instead, I suspect that it’s being compressed or pinched by a tense pelvic floor. I also have an appointment for Pohl therapy and have high hopes for it.

However, what I’m wondering is that I’d like to address the root cause. I’ve been diagnosed with congenital spondylolisthesis and hip dysplasia with hip impingement. Given the muscle pathways and the course of the pudendal nerve, it seems likely to me that the dysplasia is causing my pelvis to be misaligned and constantly tense, which is pressing on the pudendal nerve. The neuro-urologist doesn’t believe this. His treatment approach is to regularly treat the nerve with Botox (I’ve read many posts about this here), but personally, I’m not in favor of that. Of course, this is very individual. I’m just wondering if there might be others here who also suffer from hip dysplasia or another hip condition that affects the pudendal nerve? The orthopedic doctors are very divided on this. Some say it’s possible, while others are less convinced. I’m facing the possibility of a pelvic osteotomy, and I’d be more motivated if my pudendal nerve pain could improve as a result. So, my question to you is: Has anyone had a PAO (Periacetabular Osteotomy) and experienced an improvement in pudendal neuralgia?

Furthermore, my sperm analysis results are unfortunately not very good (reduced fertility). The neuro-urologist and urologist also believe that there is no connection to the pudendal nerve. However, I’ve seen some studies that do suggest a connection, even if it’s not very strong. It certainly makes sense that if I have constant pain in the genital area, my sperm analysis wouldn’t turn out great. Still, I’d be interested to know if there are others here who are affected by this and understand the connection?

There have been pudendal nueralgia patients on this forum with hip problems such as femoral acetabular impingment causing their pudendal nerve problems. Some of them have gotten better from surgery to repair the impingement.

Maybe you could get an opinion from a physical therapist who specializes in the pelvis who could tell you if your pelvic alignment is a problem, as you suspect.
