how are you doing?
I had very good cca 6 weeks, from the end of March till the half of May, approximately. Far from being cured, but was definitely better....compared to some other sufferers, I was funkcioning pretty normal, with no major limitations (if I dont mention that I cannot still play tennis). But otherwise pretty ok. I was for 6-7 weeks in great mental state, having finished the book I was writing with my colleague etc. Mental state and psychology is enormously important, but this is known very well.
Last 10 days I have to report some worsening, maybe related to long sitting sessions in the frame of my work duties. Burning pain at the base of penis came back, sore pelvic muscles, pelvic discomfort etc. I had this setbacks and improvements several times in the past, and always the improvement came.
Back to my treatment:
-low dose Amytriptylin and Neurontin
-Phlogenzym (which is enzymatic treatment, very effective after injuries, post-fibrotic swelling etc.)
-PT with internal trigger points release, stretching the core, adductors and hamstrings (every 10 days a visit at PT)
-light PT at home, mainly light stretches and tennis ball masagge in the pubic region.
Hope that current worsening of burning pain is only intermittent and will fade away.
Keeping the fingers crossed for both you buddies.

I had Dr. Dellon cut the perineal branch of my pudendal. He had to first locate the dorsal branch. The report states that it was hard to find, but when he did he found a portion that was yellow and appeared to have been being compressed. In the report it says he performed "resection" (which to me sounds like he cut it, but he says he did not) of the dorsal branch.

A word to the wise: Do not fly the next day after surgery in this area. I did. My penis and scrotum swelled up like toy balloons because of the pressure change. I cannot say if it was the resection or the pressure, but a couple weeks later I started experiencing penile shrinkage. Embarrassingly so. The only other possible explanation is that in the midst of all this, the anastamosis from my prostatectomy 3 years prior (cancer) started moving. I have read that can pull the penis in towards the body. It typically shows up right after the surgery though.Tried everything you can imagine to rectify this issue. Talk about adding insult to injury!!!
