I have decided to wait until after my Potter MRI in New York and my Dr Hibner appointment in Phoenix at the end of October before I go to see dr Azsmann.
I will definitely post what I find out and hope you find success with Dr Azsmann.

I thought I'd give you a quick update of my Dr Potter MRI .After a grueling flight to New York City from the Inland Northwest she says I have no scar entrapment of the nerves but I do have 'prominent varices coursing the dorsal nerves of the penis bilatrally as well as in the inferior Perineal branches of the pudendal distribution.A focal varix is noted on the right effacing the fat around the pudendal nerve.A prominent varix is noted in the left inferior margin of the Alcocks canal.'
I don't know if these varices are my problem or not but I did read earlier that Dr Azsmann confirmed it could be. Any of your advice would be welcome.

There have been a few people (mainly men I think??) who have reported varices as the only thing noticeable on a scan. take a look at Violets first post here Earl. It will give you a better insight into this particular problem

Fall 2008. Misdiagnosed with lumber spine problem. MRN June 2010 indicated pudendal entrapment at Alcocks canal. Diagnosed with complex variant piriformis syndrome with sciatic, pudendal and gluteal entrapment's by Dr Filler 2010.Guided piriformis botox injection 2011 Bristol. 2013, Nerve conduction test positive; new spinal MRI scan negative, so diagnosed for the 4th time with pelvic nerve entrapment, now recognised as Sciatic, pudendal, PFCN and cluneal nerves at piriformis level.

Unfortunately patient reports of surgery results for varices along the dorsal nerve aren't very positive so far but there may be successes that we just haven't heard about.

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

Were you seeing that varices were not confirmed to cause compression or just that dorsal nerve surgery in general has not shown to be successful?

Following is an email exchange between Dr Azsmann and i. His response is listed here ahead of my query because of how I copied it from my emails. I feel this shows why doctors quote a much higher success rates . I believe there are actually five or six failed dorsal nerve surgerys on the forum not just four.

On the pudendalhope network you will find lots of patients that have improved after dorsal nerve surgeries by me who are from your country. I am not sure whether I have the right to disclose their identity- so why don't you ask on their forum. Obviously not everybody improves to the same extent, but for sure I have not made anybody worse. Otherwise I would stop this treatment.
It is impossible that nerves that have been irritated for so long heal immediately and 100%. So improvement to the point where people can get on with life without too much distraction from this problem suffices for most.

I wondered how many of these dorsal nerve surgeries you've performed ?Also for how many years? I am a part of a forum with others having PN problems and there are 4 men who've had the dorsal nerve surgery without success and some even worsened from surgery.I wish at least some on the forum would have had success .I am considering possibly the middle of November to come but would like to research some more.

I'm not sure who Dr. Aszmann is referring to in his e-mail -- I don't remember anyone posting success stories from dorsal nerve surgery.

dr. Aszmann is very experienced surgeon and I have only the best impression from the exam done by him.
This is not true, that there are no success with dorsal nerve decompression surgeries. The results vary widely - from excellent to pretty dissapointing.
Surely that this operation has high risk profile. This is the main reason I am still hesitating and may never try it.
After the summer break, I feel somehow better, having experienced really relaxing and beautiful summer in our weekendhouse in mountains etc.
But my situation is not fully cleared. I am planing to try that PSSD test by dr.Aszmann and some new physical therapy cycle.

early 2010- major flare-up, chronification
february 2011 - ESCW wave. major flare-up, lasting 5 months
february 2012 - diagnosed CPPS with irritation of pudendal nerve, hypog. plexus block

2014-2017 better periods interchanging with heavy flare ups
2018 first long remission (several months)

Its great that you are doing some better- at least not worse !If I were you I would definitely not do surgery yet.
I'm encouraged to hear there are successes with the dorsal nerve surgery. If you would know of anyone who did I would love to hear from them. I still feel that my problem may lie with the dorsal nerves. I just feel I can't wait around till my penis is completely numb. Not being able to sit is awful enough but I'd take that over the numbness. I can't see I have anything to lose . I'm not working or driving or anything . Every day is just a battle to see the good in anything in this life. I'm hoping Dr Hibner has some answers for me in a couple of weeks.
