Sorry it took me so long to see your posting on this thread but I had pelvic surgery with Dr. Howard on the 17th and haven't been on computer much. Yes, every prospective patient has to fill out about a 12 page questionnaire to apply to be a patient of his. He only sees patients 2 days a week every other month. I think he's still a professor at Univ. of Roch. too. There is a place to write an essay type reason on why you want to see him so make it good. Let him know you have PN and are desparate for help. I suspect he turns down common ob/gyn problems. Also specify that you want Dr. Howard himself not the regular ob/gyn doctors. I waited for 3 weeks and called and was told he would see me right away. It would be easier getting an audiance with the Pope than an appt. with Dr. Howard but he's the only one in the area familiar with treating PN/PNE. He will probably send you to Dr. Westesson (neuroradiologist) for guided nerve blocks. They aren't bad at all as he will numb your buttocks so don't fear that. My best advice is to catch his interest in the essay portion so he knows only he can help you. Things move slowly so get started right away. By the way, where are you from? I live in Dansville.
