Hi Mugofwater,

Just like Violet and Stephanies, my symptoms were bilateral, but I had extra sensitivity on the ischial spine on the left, and I had stabbing sharp pains periodically inside slightly above the pelvic floor on the left side. Maybe 1/100 times I'd get something like that on the right, but it was overwhelming on the left. So it always seemed very clear to me that it originated on the left. But the burning pain traveled across the perineal branch of the nerve throughout the pelvic floor, so I was always had bilateral burning. Violet's description of having been hit with a bat matched how it sometimes felt too, and that was a bilateral feeling.

Thanks for all the helpful replies all. I'm still struggling with this unfortunately, but I feel like some of the general posterior thigh burning has gone down. If I sit on a hard seat, what jumps out to me immediately is the pain on my sitz bones (bilateral). I can very clearly feel those pelvis bones against the chair, and this is way more prominent than any feelings in the perineum, genitals, or rectum. If anything, it radiates a bit more outwards (laterally) and down the thigh.

So I'm now still as confused as ever. Does this sound like pudendal neuralgia? Is the sitz bone pain a common symptom? Is it maybe another nerve involved like PCFN? Though that seems even rarer than PN.

My pain wasn't directly on the sitz bone (ischial tuberosity), it was more medial (to the inside of) the sitz bone. Has your doctor ruled out ischial bursitis? https://www.ncbi.nlm.nih.gov/books/NBK482285/

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

My doctor doesn’t think it’s ischial bursitis. I think the pain distribution seems closer to PCFN but I don’t know how I would have it bilaterally given that unilateral is already rare enough.

I recently saw an article where they do decompression of PFCN and PN at the same time. I guess it can be tricky figuring out which you have sometimes.
