Burning Thighs and Feet with some PN pain - scared

44 y/o male. Long history dealing with PN pain dating back 16 years. Over past year, my symptoms finally dissipated significantly (I would say 95% resolved). I finally felt like I had my life back and was able to do things that I had not been able to do in a long time w/o pain. It was wonderful). Over the past 10 days, however, I started to feel a burning in my thighs and feet and a slight uptick in pain in my scrotum/penis. Not too much in the scrotum/penis, but enough to worry me because of my history.) Anyway, the bilateral thigh and feet pain is unrelenting and brutal. I cannot get a break.I don't know what is causing it and it is difficult getting to see a doctor in person because of COVID19. I need relief, but my bigger concern is that this is not PN related. A few doctors that I was able to speak with do not think so. They believe that there is just some "cross-talk" with the pudendal nerve and that the pudendal nerve is not causing the leg and feet pain. I am scared, in pain and unsure. This is a nightmare. Does anyone have any ideas for me.

I should have been more specific in my question. Does anyone think the doctors could be correct in that the other nerve or nerves that are acting up and causing the thigh/feet pain are "cross-communicating" with the pudendal nerve and bothering it? and therefore, the root of the problem is not necessarily the pudendal nerve? so, if i relieve the issue with the other nerve/nerves, the pudendal nerve, in theory, should calm down as well?

Also, does anyone know what could be causing the burning thigh and feet pain? and what could help stop it. I have been prescribed a steroid pack as well as topamax. And have upped my dosage of gabapentin. I need relief and so far nothing is working. I have been on the verge of going to the ER a few times now, but each time have backed off. I'm not sure what they could do for me. I just don't know what my options are. I've cried more times this week than I have in the past 16 year of dealing with PN. This is unbearable.

Hi Dutch,
My name is Billy from Thailand. My pain also runs down from my groin to my feet. Its constant pain even when im lying down. Everynight i rely on Gabapentin and also in the morning. 1200mg in the morning and most of the day bedridden living with mom and dad. I would kill myself after they are dead. Probably pay a visit to the shooting range and get over with it. I feel your pain Dutch. You are not alone. You can take as most as 4000 mg of gabapentin a day to relieve the pain but the side effects are suicidal thoughts. Beware of that. I cry everyday too since taking them. The pain u get on your thigh and feet are pudendal related. But if u want to be sure u can get a lumbar spinal mri. Take care dutch

Hi Dutch, I'm so sorry to hear that your pain (or at least some kind of pain) is back. That does sound like an extreme case of nerve cross communicating, so I'd be skeptical of that interpretation too, especially if you didn't have that issue when you had pn. But, I'd also be reluctant to go to an ER right now in most big cities. I think I'd probably try to get in to see a pcp and get a referral to a specialist. Maybe a neurologist?

Hi Billy, I'm so sorry to hear about your pain. That sounds very difficult. Do you use ice and/or heat? I'm wondering if you could wrap heating pads around your legs to relieve the nerve pain? I find both heating pads and ice very useful. That might allow you to lower your gabapentin dose somewhat.
