HELP... epidyd pain bilateral and now hot poker in perineum

I don’t know if anybody’s had this experience but it started about three months ago with what I thought was epididymitis and was treated by to urologist for both epid and prostatitis the pain was on the epididymis first on the left and then on the right going back-and-forth with the pain the work up has been to see a Doctor Who specializes in what’s called micro denervation of the spermatic cord I had a block which was equivocal and I’m on multiple meds now my concern now is that I developed peroneal burning and constipation without a bowel movement in over six days on Senokot and I don’t know what the next step is I presume I should get an MRI I don’t know if my problems were first Still this neurologic or genitofemoral nerve intervene in my scrotal sac or I have a pudendal nerve problem that can cause symptoms such as that that is now finally developed into a burning sensation of course the constipation could be causing irritation of the piano nerve. please anybody with suggestions or if they’ve experienced anything like this before?

I should add that my problems do seem to be more of an anatomical disruption of the left testicle?. which seems to have fallen much lower than right. I’m very scared and don’t know if I stretched the cord somehow by this and/or the innervation of the left sac. Maybe this is the wrong forum but n o urologists can give me a good answer and I’m searching for anyone with a similar problem ? wish there was a chronic scrotal pain forum similar to this....
