I was diagnosed with proximal hamstring tendinosis by MRI, had 2 cortisone injections and each time felt worse. The main source of my pain is near the sit bone but more towards inner thigh. The pain is constant and hurts to sit and stand, the skin is hypersensitive to touch, its a stabbing or pinching pain. I have no perineal pain or other genital pain or rectal pain.i am seeing a pelvic physical therapist, just a few time so far.The orthopedist still thinks hamstring, but this pain have been the going on 5 months, and it’s getting harder to function.
Does this sound like pudendal neuralgia ?

Sue,
The pudendal nerve innervates the rectum, perineum, and genitals. Without symptoms in that distribution, I would certainly consider other diagnoses including ones involving the hamstring or possibly a different nerve. Can you be evaluated by a gyn or urogyn with knowledge of these issues?

PN started 2004 from fall. Surgery with Filler Nov. 2006, Dr. Campbell April 2007. Pain decreased by 85% in 2008 (rectal and sitting pain resolved completely), pain returned in 12/13. Pain reduced significantly beginning around 11/23.

Thank you for your reply, I have seen a gyn who didn’t think it was pudendal neuralgia, not sure how familiar she was though. But I agree it does seem some of those other areas would be involved. Just am never comfortable, especially sitting.

Saw a Urogynecologist, said he could not rule out PN, and I should see someone else with more experience in that. I have pain all day now, sitting or standing, getting really tough to work.

Can you see a pelvic pain specialist who can give you a better idea if your pain is in the distribution of the pudendal nerve? Maybe a pelvic PT?

I don't know for sure but as close as I can tell from your description of the area of pain it sounds like the same place my pain was -- just inside the sit bone area towards the inner thigh. Dr. Bautrant told me the most of my problem was from the perineal branch of the PN.
PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.
