9 Months Post TG

Hey Everyone,

I am 9 months Post Op with Dr. Conway.

I hate to say it, but i am definitely worse at the moment........

Just thought i would update on here for anyone interested.

I know i have up until 2 years, but as of now i still can't sit, i still get pain with sex, still have numbness, and now even standing for too long is bothering me a long with worsening of my lower back pain.

I am starting to think i have SI problems but not sure if it is from surgery or if i had SI issues this whole time and it was a big part of my entire problem.

Honestly don't know and am way too exhausted to do anymore detective work after 5 years with this.

One thing that has improved is that even with numbness and all the pain, my erections have been much more normal now.

Hi Joe,

I'm sad to hear you aren't feeling better. Do you know if Dr. Conway cut any ligaments during surgery and if that might have contributed to SI problems? I wonder if some prolotherapy would be helpful if the SI joint or ligaments are a problem. When I had prolotherapy into the SI joint from a physical medicine and rehabilitation physician, she put just a little bit of steroid into it so that insurance would pay for it but not enough steroid to negate the effects of the prolotherapy.

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

Hi Joe,
Its been 10 months since my surgery. I'm in very similar position as you. My lower back pain has worsened over the time.
However, I think its other way around that SIJD caused pudendal nerve compression, although severing the sacrotuberous ligament makes it worse.
I've been working lower back specialist and he has put me into physiotherapy cum rehabilitation for my core and butt muscles.
First appointment with physio, I wasn't able to find my gluteus (butt) muscle, let alone clench or squeeze it. I was using my hamstrings and thigh muscles for compensating those.
Prolotherapy is an option but my specialist want to make sure that frugal forces( core muscles and butt muscles) are actually working .
For me erections are bit tricky, some days i would normal erections but on other days it's not the case. Only noticeable improvement form surgery has been reduction in numbness and tingling sensation.
After intercourse my lower back becomes very stiff and sometimes pain as well.
Can you please explain what 's you experience with erections before and after surgery.

I have also done a lot of core strengthening and butt strengthening exercises which definitely helps with pelvic instability and SIJD dysfunction. I think the secret is to start out very slowly and gently so that you know what exercises you can do without flaring things up. I didn't start anything until almost a year after surgery, just to make sure I had given enough time for the nerve to heal. My PT had me do some gentle exercises to get some muscle groups working again because I had been guarding the pelvic area for so long due to pain that I had forgotten how to use some of those muscles. Also, a TENS unit can be very helpful in getting some of the pelvic floor muscles to calm down after being tense for so long.

Couldn't agree more with you violet.
I too feel when I'm doing butt/glute exercise, nerve flares up a bit. But thankfully its only short lived.
