S1 (sacral) insufficiency fracture

Post by mgail »

Hi everybody.

It's been some time since I posted anything. After battling every symptom of "pudendal neuralgia", going through various treatments including intravaginal botox injections (twice with increasing doses), intravaginal nerve blocks, Interstim trial, various drugs, Tramadol, gabapentin, etc., suffering with severe burning buttock pain with sitting for more than 6 years, my last option was to have a presacral neurectomy by Dr. Guerette in Richmond, VA. I balked at this after doing my research. After dwelling on things, I located a top neurosurgeon (this is the second neurosurgeon I have seen, the first one only got a lumbar MRI 3 years ago and my spine was not in bad shape), Dr. David Vincent in Virginia Beach. After talking with him, he asked me if I had ever had a bone scan or a pelvic MRI. I told him I had not (after seeing another neurosurgeon, orthopedic surgeon, etc. who never checked my pelvis). He ordered a bone scan, a pelvic MRI and a lumbar MRI. The bone scan that was done is a nuclear bone scan where radioactive dye is injected, you wait 2 hours, and then come back and they do a scan similar to a CT scan (this is not a bone density test)... Days later I received a phone call from his nurse stating that my bone scan showed an S1 (sacral) insufficiency fracture (I do not have osteoporosis, and am in good shape, not overweight, but have had severe trauma to my pelvis). My next appointment to go over the findings is not until the day before Thanksgiving. I do not know the results of my MRIs yet. One of the treatments for insufficiency fractures in the sacrum is a sacroplasty, which is injecting methylmethacrylate (glue) and basically gluing the fractures and stabilizing the spine. I just want EVERYONE TO BE AWARE, if you have had severe falls or trauma to the pelvis and then developed the symptoms of pudendal neuralgia, HAVE SOMEONE DO A nuclear BONE SCAN AND CHECK FOR A SACRAL FRACTURE. The symptoms are very similar. It could be a fracture, not your nerves. DON'T WAIT YEARS to have someone do a bone scan or other appropriate scans to rule out a fracture. I am praying that I can get my life back after years of suffering, but as everyone knows, pain is so debilitating and it's hard to keep being positive. I am just glad at this point to finally have someone actually show radiographic evidence of an abnormality and going forward with some type of actual treatment.

Interesting, please keep us posted on the results of your studies and the action that your doctor suggests.

Hello dear, I was wondering what was your outcome? Did you go the sacroplasty route? Will appreciate if you share how u doing :)

Mgail,

I'm also curious about how things went with this procedure.

Thanks,
April
