PUDENDAL PROBLEM - HI FROM SOUTH AMERICA!!

Hello to everyone,

I am a Peruvian citicen.

As most of you I have pain when being sitted.

I have an entrapment of the pudendal nerve in the base of the penis and probably in Alcock canal.

Nextweek I will try shock wave therapy. I hope it can soften scar tisue or fibrosis I could have , increasing space in the canals and therefore feel a little better.

Another option I have is a nerve block. I would greatly appreciate if somebody can tell me who is THE BEST doctor to have a pudendal nerve block in the USA.

Welcome to the forum, Cholo. I don't know where the best place is to get a nerve block in the US. There are many docs who do this and I'm sure not all of them are listed on our list of docs. Typically docs who specialize in pain management or interventional radiology give guided nerve blocks. Are there no docs in South America who do this?

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

Hi Violet,

We have doctors that do nerve blocks here, although I am looking for a doctor who has done it in the alcock canal.

Peruvian doctors do not have experience in that anatomical region.

Maybe there is not the best doctor in this field , although I would greatly aprecciate if you can tell me some doctors whom you heard were succesful in doing nerve blocks in the US.

Cholo, it depends on what you mean by successful. I really don't know of very many cases where nerve blocks gave permanent relief. There is always the potential for a worsening of symptoms from nerve blocks too. I can see some value to having a nerve block for diagnostic purposes and I would certainly have the typical series of 3 blocks before considering surgery but nerve blocks are not likely to be the magic treatment that is going to cure you.

I don't think I can say which docs on the list are the best at nerve blocks. I think most of the ones on the list of PN docs can give an image guided block into alcock's canal or they work with someone who can. Castellanos and Conway on this list http://www.pudendalhope.info/node/58 would as well as the team at Rochester, NY and Dr. Filler.
