Hi everyone,
I am new to this site. I have been battling pudendal neuralgia since the birth of my fourth child almost three years ago...I tore to the side and had a hematoma...right in the spot where the dorsal nerve comes out over the pubic bone. According to Dr. Dellon, that is a vulnerable spot. My nerve pain is isolated to the dorsal and perineal branches. I've never had any rectal symptoms, except when the spasms get going...and then I hurt in my sacrum, down my legs, everything. Botox seems to help with that part. Right now, I receive regular steroid and botox injections, and take cymbalta, narcotics, and diazapam/baclofen/lidocain vaginal suppositories.
My question is, has any one had the anterior approach to surgery? Has ANYONE had success? Any advice? I want my life back!

I know one individual who had the anterior approach with Dr. Aszmann. Very similar situation to you, really bad tear during vaginal delivery. He found a huge neuroma, I think on her perineal branch. She was improved after the surgery but before this surgery she had developed Complex Regional Pain Syndrome which complicated her recovery. She was still having a lot of problems even after Aszmann surgery and had cryoablation of the pudendal nerve about 1 year ago and is doing great. I do not know if Dr. Hibner has done any anterior approaches, I know he went a couple of years ago and learned the surgery from Dr. Aszmann.
If you do a search on Aszmann in the forum I know a couple of others have had surgery with him. You could PM them and see how they are doing.

Thank you SO much for getting back to me. I had not even thought about cryoablation, but it sounds promising. I researched it, and called Dr. Prologo's office this morning. Hopefully I will hear back from him soon.

My friend was very impressed with Dr. Prologo. Like I said she is doing quite well since her cryoablation.

I just talked to Dr. Prologo over the phone. He is going to talk to my pelvic pain doctor here in Birmingham. Fortunately, we're only a couple of hours from Emory, where he works. He said he's done about 25 pudendal cryoablations so far, and that he has had good success in patients with vaginal pain, but not with peri-rectal pain. I was hoping that he could target just the perineal branch, and spare the dorsal branch, but he can't. I would love to be able to preserve sexual function...but like most people on this site, that is absent anyway, and I would trade sexual function in a heartbeat just to be able to get out of bed in the morning. I am going to talk to my regular doctor about it at my next appointment in March. I will keep you updated. Again, thanks so much!

What kind of approch uses dr. Aszmann? Who except the prof Tibet Erdogru does the laparoscopic surgery?
