I started suffering from left sided deep buttock pain several months ago - no leg/hip pain at all. It started as a dullish ache in the bottom of my left buttock (saddle area) and over the months the pain has become much worse and at times the pain goes into the genital region too.
On waking in a morning I'm usually fine but by the end of the day the pain is unbearable and it hurts to drive, and sit for too long. I can only get rid of the pain by lying down but it still takes some time for the pain to subside (a couple of hours usually).
I have had Xrays and an MRI which showed nothing unusual. My consultant recommended a steroid injection in my SI joint, but I really don't think it is back related and he said it was more trial and error than a diagnosis - so I haven't actually had the injection as yet.
I have recently been seeing a gynaecology as was experiencing heavy bleeding and left sided ovary pain. A few weeks ago I had a diagnostic laparoscopy where they removed a large cyst from my left ovary and also discovered severe ovary veins. I am now booked in for a 2nd laparoscopy to remove the veins and ovary on the left side.
I have obviously asked my consultant if the veins could be the reason for the buttock/genital pain and he is unsure but says it is a possibility. I also get a lot of urine infections.
I have googled for hours and cant seem to find anything that fits my symptoms well. Strangely I have just been to Egypt where is was really warm and the pain was much better, within a few hours of returning to the UK the pain is back.......?
Has anyone else had similar symptoms and it been due to ovarian veins or similar? Could it be PNE?

Sometimes it's hard to zero in on the exact cause of pelvic pain and it may take a bit more diagnostics for you to figure out the primary cause. For sure some of your symptoms sound similar to pudendal neuralgia but there can be other things that cause similar symptoms. Do you know if your pelvic floor muscles are tense via pelvic exam? Is there any tenderness along the course of the pudendal nerve?

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.
