Pudendal Nerve Entrapment...

Post by Andy_Pablo »

Hi. Im Andy. Im 35 & have Pudendal Nerve Entrapment from cycling.

My story started after my 13 year relationship ended & I lost my home, I threw myself into a mixture of alcohol, cocaine & my job... Working 15+ hour days for five days per week & then drinking & snorting myself into oblivion for the other two, was how I lived my life for months after my split. I finally decided to sort myself out about July 2013. Was going to the gym, running & cycling. I had always been extremely active with football, running, kickboxing, weights, surfing, etc, but I had never cycled. I had friends who cycled a lot, so I figured I would just join them. I picked up a road bike & set about regaining my fitness. Started off with small rides of about 15-20 miles & regained my fitness levels pretty quickly in a few weeks, & as I have always had a desire to push myself, I started to do 20-30 miles a few times in the week & my last cycle was a 50 mile ride. Unfortunately, as a complete novice, I was totally unaware that a bike needs to be set up for individual body size. My bike was set up completely wrong for my body shape/size & with doing so much, so quickly, I did damage to my pelvis & that crushed & trapped nerves. This has caused excruciating pain which starts from my hips & distributes itself down through my groin, to my knees.

It took a couple of months bad form from the cycling to make the pain become constant. I had ignored it to that point thinking I should just "man up" & it would sort itself out over time. Fourteen months on & it has become worse & worse. It cost me my dream job as a graphic designer in the sports industry. In the space of three years, I had lost everything & was in pain I didn't realise existed so I looked for a way out. I was helped by my brother to get through the worst of my slide into my dark place.

As of today, I am in the process of receiving Pudendal Nerve Blocks from Dr. Greenslade in Bristol & hope it can settle itself over time. They don't really help much & recently, things are so bad that I cannot escape it at all. I am told that it is unlikely that the injury will ever fully recover & the most probable scenario is that the pain becomes "manageable. I am practically housebound fourteen months after the initial injury & have had exactly six blissful days which have been what I would class as manageable.

I find it incredibly frustrating to have gone from being extremely active, to having to ask for help with the simplest of tasks. I also find it very difficult coping with not being able to continue with my career.

I just hope that things improve, because Im not sure I can take much more of this...

I am taking Lyrica at the moment, but they dont touch the pain anymore. I was on other pills, but had to stop due to complications.

I havent tried physiotherapy, as I have no idea where or how to go about doing it... I will speak to Dr. Greenslade about it during my next appointment.

Surgery hasnt been suggested yet. I have my final nerve block on 25th October & Im hoping that settles things a little... After that, I will have to see what course of action to take next. I need this to settle...

My physio is in London Maria Elliot, and she says men recover pretty well with physio, you need to do some research on the web to find a local physio to you who specialise in PN, NHS is pointless you need to go private. Also there is a forum in the UK worth joining as the lady who helps run it is very knowelegable about what's in the UK, The forum is called Health unlocked, you would want the pelvic pain part of it, there are some guys on there.

Andy,
I assume that you had a full workup by a urologist. No bacterial prostatitis etc. If it is the case, I don't think you should dismiss the fact that you had had a very loaded emotional period prior to that incident. The fact that you had fairly good days in the middle of a terrible period may be a telltale.
You can see if my story and recovery resonate with your experience:

2002 PN pain started following a fall on a wet marble floor
2004 Headache in the pelvis clinic. Diagnosed with PNE by Drs. Jerome Weiss, Stephen Mann, and Rodney Anderson
2004-2007 PT, Botox, diagnosed with PNE by Dr. Sheldon Jordan
2010 MRN and 3T MRI showing PNE. Diagnosed with PNE by Dr. Aaron Filler. 2 failed PNE surgeries.
2011-2012 Horrific PN pain.
2013 Experimented with various Mind-body modalities
3/2014 Significantly better
11/2014 Cured. No pain whatsoever since

There was no bacterial prostisitis, but I was given antibiotics nonetheless...

My emotional state has been in turmoil for over three years since my father passed away. My inability to deal with that is what cost me the love of my life. I still break down to this day over everything I have lost over the past three years. Now my fitness is gone, I am struggling to see any positive future... Every day is a struggle just to exist...

That is standard procedure. Most of us guys went through it.
Don't you think your emotional state has more to do with your pain than the badly adjusted bike? After all many people ride badly adjusted bikes without problem. If you think there is a connection, have a look at my recovery story or the "TMS wiki".

I think my pain is physical. My emotional pain is real, but that is a seperate entity...

Hi Andy

I am female, aged 59 - everything happened to me after a very stressful period - emotional stress will make your pain feel worse - because this pain restricts you it also makes coming back to normal life after a bereavement very difficult. Your pain and your other feelings will all get muddled up.

I see you live in Devon - i do also - i have seen a physio recommended through my local pain clinic - i emailed her again recently and she says that she is aware of PN issues and chronic pelvic pain - she treats men and women - i have also been to see Dr Greenslade. I don't know whereabouts you are but If you would like anymore information i could send you a personal message - just reply on here and i will contact you by pm.
