My pain doc is of the opinion that, since the nerve block he did where I was pain free was away from the spine, that the source of pain was PROBABLY further away from the spine than the injection point. He said he has always wondered if enough of the numbing agent was able to seep back toward the spine, if that would be enough to numb pain from the spine area. My L5-S1 disc has a bulge, butI have decided that my pain must be further away from the spine than where the nerve block injection was done.
I went to DiagnosisPro.com, a very interesting website that provides differential diagnosis techniques for single or multiple symptoms. I came up with two potential causes for sciatica and unilateral testicle pain: (1) Piriformis Syndrome, and (2) Ischial Tunnel Syndrome.
Both are diagnosed by EMG using something called the FAIR Test - Delay of H-reflexes in flexion. adduction and internal rotation.
Apparently, 15% of all people have their sciatic nerve pass through the piriformis muscle. (I've already tried botox in my left piriformis muscle but got no relief.)
Ischial Tunnel Syndrome involves entrapment of the sciatic nerve as it passes by the ischium. "The pudendal nerve may be separately involved". Reference http://www.sciatica.org.
I am going to see an osteopathic doctor next week.
Has anyone heard of the FAIR test? Had it done?
Is EMG just a nerve-conduction study? I don't have much faith in those. I almost tried the PNMLT, but then read that the test was inconclusive (and painful).
Has anyone tried the F.A.I.R. Test?
Has anyone tried the F.A.I.R. Test?
Left testicle pain since 2008. Left sciatica 2010-2012. Failed left epididectomy, orchiectomy, botox injections, nerve blocks and internal physical therapy. Genital branch of genitofemoral and perineal branch of pudendal nerve cut. L5-S1 microdiscectomy cured sciatica. Dorsal Root Ganglion nerve stimulator failed to help and was removed. I have had 4 pudendal nerve blocks, two from Dr. Poree worked for 2 hrs. The ONLY break from pain ever.
Re: Has anyone tried the F.A.I.R. Test?
Hi Jon,
I have heard of the FAIR Test, also known as a "provocative EMG". I haven't had it, and the only people I know of who have were being tested for Piriformis Syndrome.
Best wishes - please let us know how you make out!
Regards,
Karyn

Ultra Sound in 03/08 showed severely retroverted, detaching uterus with mulitple fibroids and ovarian cysts.
Pressure and pain in lower abdomen and groin area was unspeakable and devastating.
Total lap hysterectomy in 06/08, but damage was already done.
EMG testing in NH in 04/10 - bilateral PN and Ilioinguals
3T MRI at HSS, NY in 09/10
Bilateral TG surgery with Dr. Conway on 03/29/11. Bilat ilioinguinal & iliohypogastric neurectomy 03/12. TCD surgery 04/14.

The osteopath had to look up the test and he did not think it was necessary. Another dead end!
