SIJD question
SIJD question
I never thought about it but I started noticing that when I lift my left leg to sit at our picnic table (while I could sit) my left hip would pop. I tried it again tonight without the sitting of course and it popped each time- over and over. I think my burning level went up in my groin. What I mean is, even with the 1800 mg. of gabapentin, I can feel a burn but it's a 1/10. I know without the med. I would now be a 7 or 8 if not worse. My piriformis muscle bothers me now on the left side. Started about 1 1/2 months ago, probably from always putting weight on it. Who do I see about this? An orthopedic doc? I don't have pain in my hip and only a slight burn/soreness in the muscle. How is it that I PN symptoms, piriformis syndrome, and SIJD all coming on one after the other?
Symptoms began 6/27/12. Diagnosed with Vulvodynia.Estrace and Lidocane with no relief.
Uretha enlarged Aug. 2012. MRI of Lumbar in Sept. 2012.Normal. Did MRI 3 Tesla of ischial spine to Sacro Spin- Normal. Houston-Nerve Block 10-31-12 & MRI (MRN) entrapment of perineal branch, more on right side. Dr. Hibner- block of perineal branches and sclerozation w/relief over 1 month. SIJD/Piriformis Syndrome Left Side. Gabapentin 2400 mg.

Re: SIJD question
You see a PT Manual Therapist or another PT who has experience in pelvic joint dysfunction (try pelvic floor PT's 1st, but if none in your area manipulate joints then you might do better asking sports PT's).
No Orthopedists, although maybe an Osteopath. Orthopedists are not hands - on. Mine took x-rays and said there is nothing wrong with my pelvis. I asked him, "why does my PT say my pelvis is so twisted that one leg is 1.5 inches longer than the other, and you say there is nothing wrong?". He said, "Because we look at objective things and they look at subjective things" - which is absolute crap. They look at imaging studies, which are still pictures in a neutral position and will not show a movement dysfunction; and PT's are hands-on and are able to discern how your joints behave (i.e. are some hypermobile? Are some hypomobile, aka stuck?), and how they are aligned when you flex and extend and walk, not just when lying flat or standing straight. In other words they really assess you instead of just ordering a test.
An Orthopedist might be helpful if you suspect a labral tear or bone spurs in a joint; otherwise PT.

pelvic pain started 1985 age 14 interstitial cystitis. Refused medical care from age 17, did GREAT with self care for years.
2004 PN started gradually, disabled by 2009. Underlying cause SIJD & Tarlov cysts
improved with PT & meds: neurontin, valium, nortriptyline, propanolol. (off nortriptyline & propanolol now, yay!)
Tarlov cyst surgery with Dr. Frank Feigenbaum March 20, 2012.
Results have been excellent so far; but I won't know my final functional level for a couple of years.

Re: SIJD question
Thanks HerMajesty. I'll call Monday and check on the PT. Guess I'll need a referral for that.

Re: SIJD question
Made an appt. with an osteopath doc. I'm in a flare today. Don't know if I can handle hands-on. If anything touches my left hip or coccyx, I suffer. If I'm not making much sense, it's cause of the burning pain.

Re: SIJD question
Hopefully you will get some answers rea. I'm sure that even if you tell him/her where the pain is they will get a good idea and then of course if an area is pressed to elicit pain then certainly the puzzle should be a little easier to solve. I think it's people that have a more generalised area of pain that can't be pin pointed that have a more tricky job to decipher. At least you can give some good answers to help.
Good luck.
Helen

Fall 2008. Misdiagnosed with lumber spine problem. MRN June 2010 indicated pudendal entrapment at Alcocks canal. Diagnosed with complex variant piriformis syndrome with sciatic, pudendal and gluteal entrapment's by Dr Filler 2010.Guided piriformis botox injection 2011 Bristol. 2013, Nerve conduction test positive; new spinal MRI scan negative, so diagnosed for the 4th time with pelvic nerve entrapment, now recognised as Sciatic, pudendal, PFCN and cluneal nerves at piriformis level.
