Hip arthritis and PN

Had a ganglion impar block a few weeks back. No luck. Decided to see an ortohpedic surgeon because for years i've had minor pain and discomfort in my groin. I never paid it any mind since it paled in comparison to the other issues I have in othe places. But I saw the ortho yesterday and he says I have arthritis in both sides of my hip. I'm 34 and at a normal weight so it seems kind of young to have arthritis. Not to mention, my PN like symptoms started when I was 20. My hip problems started then too so it seems logical that one would be related to the other but I was just curious if anybody out there has anything similar. The ortho said he is 95% sure that my hip problems are related to the tingling in my scrotum which is my worst and most prevelant symptom. But I also have it in my penis and he said he'd never heard of that with arthritis of the hip. Needless to say I'm a bit confused.

Scrotal pain and tingling started in 1999. Soreness after ejaculation. Occasional numbness in Scrotum. Some urinary frequency. Symptoms disappeared for 3 years in 07 and came back in 2010. Tingling and tickling in penis 2011. Pain in buttocks with sitting started in July of 2012. Not much success with PT. Tried the usual meds with some success, but didn't like the side effects. Dr. Potter MRI revealed pelvic floor varices. Both hips opperated on in 2013 for FAI. Some help with supplements.

Hi Elmwood,
I'm sorry to hear about your hip arthritis, but maybe now you have some answers or at least some clues to your PN. I haven't heard on anyone on this forum with hip arthritis, but there are a few of us with hip issues and PN. Look under "case updates" and "related disorders" in the board index for hip topics discussions. I have hip labral tears, impingements, and dysplasia which may be a cause of mine, but I am still waiting to see the OS. I have some groin pain too, so I know what you are talking about.

What did your surgeon recommend? How did he/she diagnose you? Who is your OS, if you don't mind sharing. It sounds like they are familiar with PN, which is cool. While it is not 100% certain that PN and hip issue are linked, there is at least one clinical study of women showing the connection and anecdotal evidence. Hopefully people will do more research in that field.

I read some hip forums now and there are other people in their 30's with hip arthritis, so you are not alone.

Started with vulva itching 2/11. Diagnosed as vulvodynia and then PN in 01/12. Progressed to sitting pain and constant burning. Received 4 nerve blocks with only temporary relief, did year of pelvic PT before being diagnosed with bilateral FAI and hip labral tears 7/12. Did orthopedic PT 1.5 years. 3/13 Arthroscopic hip surgery at HSS with Dr. Kelly. Take cymbalta and lyrica daily. About 97% better than worst point thanks to combo of surgery and meds. Yay!

Hey Jackie

Thanks for your response. I am seeing a orthopedic surgeon at the Lahey Clinic in Massachusetts. I wouldn't say he is overly familiar with PN. He said that he thought my hip arthritis was the cause of my scrotal tingling but when I mentioned PN he didn't seem to register any recognition. He's going to give me an injection in the groin in a few days so I'll see what happens. I don't know what he has planned if that doesn't work. But your right, it may be a clue as to whats going on with me.

What therapies have you tried for your hip issues

How did the Ortho MD you saw know you have arthritis in the hips? Did you provide him with imaging?

That's usually indicative of an injury. When you injure a joint, it leaks synovial fluid, which in turn ossifies into some form of osteoarthritis.

Ultra Sound in 03/08 showed severely retroverted, detaching uterus with mulitple fibroids and ovarian cysts.
Pressure and pain in lower abdomen and groin area was unspeakable and devastating.
Total lap hysterectomy in 06/08, but damage was already done.
EMG testing in NH in 04/10 - bilateral PN and Ilioinguals
3T MRI at HSS, NY in 09/10
Bilateral TG surgery with Dr. Conway on 03/29/11. Bilat ilioinguinal & iliohypogastric neurectomy 03/12. TCD surgery 04/14.

Anecdotal evidence of a connection between hip problems and PN seems to grow and grow. The questions that would be interesting, well the answers to them. . . . would be;
Elmwood, is your tingling problem bi lateral and did the ortho see the same or very similar arthritic advancement in both hips?
How does he explain the link between scrotal tingling and hip arthritis?
Where is he targeting for the injection in your groin and why? Is it for hip pain?? or the scrotal problem or both?
Does he recognise that tingling and numbness are usually associated with a nerve issue?
Hope the injection does bring some good relief Elmwood.

As with most appointments, I had a dozen questions pop into my head after I left the office. But I do have bilateral tingling and bilateral hip arthritis. I feel as though its worse in my right hip. He didn't explain the connection between the tingling and the arthritis other than to say that hip arthritis can be felt in the bottom of the scrotum. I got the impression that he was aiming to ease the tingling with the injection since I don't have much pain. We'll see. The pain doctor who gave me the unsucsessful ganglion block wants to do a bi lateral pudendal block too so hopefully something works.
