Fall 2008. Misdiagnosed with lumber spine problem. MRN June 2010 indicated pudendal entrapment at Alcocks canal. Diagnosed with complex variant piriformis syndrome with sciatic, pudendal and gluteal entrapment's by Dr Filler 2010.Guided piriformis botox injection 2011 Bristol. 2013, Nerve conduction test positive; new spinal MRI scan negative, so diagnosed for the 4th time with pelvic nerve entrapment, now recognised as Sciatic, pudendal, PFCN and cluneal nerves at piriformis level.

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

Have DX of FAI and labral tears but i have never really had hip pain, only the classic PN pains. Has anyone had FAI repair surgery, with NO prior hip pain, that worked?

I'm following this with great interest too and hope your recovery goes smoothly and helps tremendously.
I had left-sided pudendal surgery with Dr. Hibner on Aug 1, 2011 after a long history of pelvic pain, and then fell on my right hip on Jan 2, 2012. Every since then I've had tons of hip, back, SI joint, knee, etc. pain. I had the MRA and was found to have FAI in both hips, a labral tear in the right, "degeneration and blunting" (??) in the left, and what may be some dysplasia in both. But the hip specialist, Dr. Larson in Edina, MN, told me to "ignore the MRA" -- definitely not pleased with that response! I'm going for a second opinion with Dr. Sink at HSS and a 3T MRI with Dr. Potter in a couple of weeks. I wish more doctors took this seriously; as a former gymnast, I'm not surprised at all that my hips, now as an adult, could be a mess and causing lots of problems.
I sometimes feel like, as a pudendal neuralgia patient, I'm treated like a leper by doctors and summarily dismissed anything related to the pelvis is being discussed.

I'm so very sorry for what happened to you in January. I can only imagine how difficult it was for you with trying to recover from surgery and then re-injuring yourself.

Ultra Sound in 03/08 showed severely retroverted, detaching uterus with mulitple fibroids and ovarian cysts.
Pressure and pain in lower abdomen and groin area was unspeakable and devastating.
Total lap hysterectomy in 06/08, but damage was already done.

Dave; I think two of the women i have corresponded with did not have hip pain but did the surgery to cure PN and it worked -- i will double check with them. Until recently i only had very mild hip pain with the PN symptoms being much worse and was told by many specialist that i didnt have a labral tear -- even though i classic symptoms -- pain with flexion and rotation. I am also a yoga teacher and had a pelvic injury as a child. My SI joint would alway be out and unstable -- but my whole leg and hip were unstable so that makes sense. My understanding is my SIJD was caused by my hip. Anyway, the one thing i have learned thought this whole process is to listen to my gut. I am 9 days post op and feel pretty good.
prolonged sitting summer -- Vulvar Burning, Vulvadynia, Urinary Frequency, Lower Back Pain, Numbness in Foot, Pain when sitting, Hip Pain
1/12 90% Better after Pelvic Floor PT and 10 mg of Elavil

Hi Helen, I think I have this prolem too! If I stand in one spot for too long I need to lean against something or put one of my feet up (on a stool for example) in order to relieve the pressure and tightness in my groin area. Do others have this? I have been told by Dr. Andrew that my pelvis is misaligned and that I have SI issues, but I didn't really get the specifics from him, just the general sense that everything is misaligned!helenlegs 11 wrote: Do you (or anyone) have any similar need to 'hang over' or rest on things to relieve what must be a pelvic pressure/gravity. . . . . .therefore skeletal? pain.

Amitriptyline and ice helps. Diagnosed with labral hip tear 2014, considering surgery.

Well I have been told by countless physio's that my pelvis is up and rotated. I know that there is no 'spring' in the worst side either. The trouble being that having the treatment to 'put me right' didn't ever work, it didn't hurt to have the treatment, but did absolutely no good either pain wise.
I just wondered if taking the pressure off, like we do, leaning etc. would always be attributed to a skeletal support problem, therefore bones and joints OR could it also be just because of a muscle problem?? I decided it was much more likely to be a skeletal thing as the muscles aren't the main framework and support of the body? Would you agree? l
I suppose if the muscles were not performing as well as they should it would then take a toll on the skeletal frame work?? Thing is I would say that apart from my piriformis muscle the rest of my pelvic muscles are fine(ish) and I've always had fairly good core strength.
I suppose if others who have suspected SIJD and misalignment say the same thing. . .. pressure off and an immediate reduction in some pain and people with tight muscles that are having an impact on the nerve (say pelvic floor) don't have this same relief it could be a pointer that pelvic realignment will help the former.

Helen, I am really not sure. I wish I could be of more help here! In my case, I think that the tightened/shortened pelvic muscles are impinging on the nerve - the questions is, are the muscles tight because of the pelvis being tilted? Can tilted pelvis cause SIJD? If so, which to correct first??
My most nagging and frequent symptom is groin pain. It feels like it's pulling. This is relieved when I lie down, so I have to take a lot of lying down breaks in the day. If I am not able to do this, I suffer with a burning pain in the evening.
Does anyone know if groin pain is more commonly a symptom of SIJD or a labral hip tear? Just wondering where I should be focusing my investigations.

A tilted pelvis is SIJD in my understanding...just different terminology. Groin pain could be hip pathology or SIJD, especially symphysis pubysis related so it's hard to say where your efforts should be focused first. If you see a PT for SIJD be sure to see one with manual therapy training, many pelvic floor PTs just aren't experienced enough with SIJD to treat it. Did you have groin pain before seeing Dr. Andrew or only after? I have horrible groin pain and burning into the upper hamstrings/adductors, but I never had this until after I saw Dr. Andrew unfortuantely.
