Well I had steroid injections initially, sweet and wonderful Karyn . They did absolutely no good whatsoever! It would be interesting to hear if anyone else has had botox into their piriformis, did it have any effect?
Fall 2008. Misdiagnosed with lumber spine problem. MRN June 2010 indicated pudendal entrapment at Alcocks canal. Diagnosed with complex variant piriformis syndrome with sciatic, pudendal and gluteal entrapment's by Dr Filler 2010.Guided piriformis botox injection 2011 Bristol. 2013, Nerve conduction test positive; new spinal MRI scan negative, so diagnosed for the 4th time with pelvic nerve entrapment, now recognised as Sciatic, pudendal, PFCN and cluneal nerves at piriformis level.

Kayran all my urinary and bowel function are normal. I did only initially have what I now no as nerve pain at the tip of my penis.That was the only pronlem I had for the first two months. That was it ,no perinium pain nothing else. That though has subsidedand never came back. Then the throbing in my piriformis started and the perinium pressure started. I do however have urethra pain not while urinating only after i sit for extended time. Ita feels as though a worm is comming through my penis. I only get a lose of sensation in my genitals when aggravating the area. And that is Minimal, I would even call it a loss of sensation. UP DATE!!!Karyn and Hellen I started to write this tread as I was in the waiting room to see DR Zhoe. We spent an hour going over my spasifics. He feels at this time to give me the pudendel block. He is highly regarded in pian managment. He a very caring doctor.shcooled in china and is a practicing Doc in the states for 22 years his specialty is trigger point injections.He has never don't a pudendel nerve block and wants to do it unguided thru the perineum as they do for child birth. I have seen and he just showed me a diagram of how its done.. PLEASE let me know your opinion whether or not YOU would be comfortable with doing the block this way.
I will be waiting to here from both of you or any on else who has an opionoin, or has had a pudendel nerve block this way.

It's your call, but I highly advise AGAINST an unguided nerve block - especially with someone who is inexperienced. Trigger point injections are much different than nerve blocks. Please view the home page of this site - you really want to see someone who is knowledgeable and has experience in this area.
Redz - I would go for the piriformis injection first. It's very possible your PN symptoms are coming from the piriformis, which can cause perineal pain. A guided or unguided PN block isn't going to be very helpful if your issue is stemming from the piriformis.

Ultra Sound in 03/08 showed severely retroverted, detaching uterus with mulitple fibroids and ovarian cysts.
Pressure and pain in lower abdomen and groin area was unspeakable and devastating.
Total lap hysterectomy in 06/08, but damage was already done.
EMG testing in NH in 04/10 - bilateral PN and Ilioinguals
3T MRI at HSS, NY in 09/10
Bilateral TG surgery with Dr. Conway on 03/29/11. Bilat ilioinguinal & iliohypogastric neurectomy 03/12. TCD surgery 04/14.

I would also say guided is preferred. Although I did not have any problems with the unguided they were more painful and the left side was not exact so I had the guided which showed clearly that the left side was the problem. Better yet if you are only having piriformis hopefully it is easier to fix and it would be good to check it out first. No one wants PNE. Good luck.

2007-08 pelvic muscles spasms treated by EGS. 6/27/10 sat too long on hard chair- spasms, EGS not work Botox help, cortisone shots in coccyx help, still pain, PT found PNE & sent me to Dr Marvel nerve blocks & MRN, TG left surgery 5/9/11. I have chronic bunion pain surgery at age 21. TG gave me back enough sitting to keep my job & join in some social activities. I wish the best to everyone! 2019 luck with orthotics from pedorthist & great PT allowing me to get off oxycodone.

Thank for the info. I choose not to have the unguided block. I will be having the Piriformis injection on Monday both sides.
However I did not ask him of he was using Botox. I think the botox would be better. Would You agree.

Thanks very much for letting us know about your decision. How are you feeling about all this? I understand how overwhelming things can get sometimes, trying to figure out what to do and who to see for treatment.
Personally, I'm not a fan of the botox. I've never had it but am aware of the mix reviews from others who have. Trying to get insurance approval for the stuff is often very difficult and time consuming. I spoke with a friend who had a series of injections to the piriformis and she states botox wasn't used. She wasn't sure what drug was injected.
I wish you the very best with your injections. Please let me know how you're doing afterwards.

Use your control F function to find all of the mentions of the piriformis in the article. There is a specific case mentioned on page 12 -- unfortunately we don't have the figures included in the article but still you may find it interesting.
If there is a facility that has a 3T MRI near you -- check out this thread by Shawn. You may be able to get an MRI similar to the Hollis Potter MRI sooner.

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

I Thank both of you for your help I will keep you updated. I see there is a donation section on the web site.
