insurance appeal for MRI

Hello, I recently had the MRI Dr. Hibner recommended in AZ that cost $4000.00, and his Dr. visit was over $500. My insurance is paying a very small percentage as out-of-network. (which it is) I am trying to appeal it for the second time to get them to cover it as in network for obvious reasons, that there is no one in network for me who treats this. Does anyone have any suggestions as to help get this covered? I am having my GP and PT's write a letter. Is there anything else I can do? I am so broke and have spent thousands on this condition and can not pay for the cost of the AZ visit. Any suggestions would be great. thanks

Suffering for 11 years after running marathons and weight lifting. Diagnosed with PN by Dr. Antolak in 2006 and Dr. Weiss in 2010. Taking Cymbalta, Gabapentin and Baclofin. Just made an appointment to see Dr. Hibner for MRI and consultation for surgery, can't get me in until Dec.
