I have seen countless doctors over the years - both urologists and GPs and internists, but none have been able to give me a solid diagnosis. Here are the facts: I used to work as a horseback riding teacher over 20 years ago. At the time, I experienced pain in my perineal area from the pounding on the saddle. Around the same time, I had blood in my semen and I started to experience erectile dysfunction. I was able to have sex, but it was painful, and at times I was unable to achieve an erection. It has been like this for 20 years. I was married and able to have sex (I have 3 children), but with each passing year the problem seems to get a little worse. It is now to the point where I need ED drugs, but sex is still painful and not always pleasant. My wife is somewhat aware of the situation, but not completely. This problem has caused both of us a lot of pain. I really do not know what to do. Based on these few facts 1) Do you think that I may have pudendal neuralgia. 2) Can you recommend treatments and tell me about their effectiveness and 3) If you can recommend any good doctors or therapists in Asia, Australia or New Zealand, Europe or the US.

I would take a look at the home pages mojo and see how your symptoms compare. There's a list of helpful treatments and doctors worldwide there too. Nerve dysfunction which PN is, can give rise to numbness, burning and stabbing pains which are quite different to 'normal' aches and pains (although they do occur too ) I hope you get some answers.

Fall 2008. Misdiagnosed with lumber spine problem. MRN June 2010 indicated pudendal entrapment at Alcocks canal. Diagnosed with complex variant piriformis syndrome with sciatic, pudendal and gluteal entrapment's by Dr Filler 2010.Guided piriformis botox injection 2011 Bristol. 2013, Nerve conduction test positive; new spinal MRI scan negative, so diagnosed for the 4th time with pelvic nerve entrapment, now recognised as Sciatic, pudendal, PFCN and cluneal nerves at piriformis level.

Mojoback, do you still have blood in your semen? That is the one symptom you are describing that I've never heard of with PN. If you don't have it anymore then maybe you could associate it with the initial injury which healed (except for the nerve). You could have some scar tissue that's still giving your nerve grief. I'm sorry this has been so difficult on your marriage -- people here will understand.

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.
