The orthopedic Dr I saw a couple days ago suggested that if pain symptoms don't subside as a result of the surgery, I should look into and treat possible joint dysfunction as the next step in trying to find relief from all of this. The thing that is a bit puzzling to me is not seeing much mention of Joint dysfunction as a likely cause for PNE like pain on the forum...the Dr mentioned that pain caused from joint dys function can very well present itself as typical pne symptoms such as pain in the rectum. It sounds like it's worth a shot to me, what I'm wondering however is where to have this done. I could find therapy in LA with Angela Sprulli or with Julie Sarton (irvine) yet it seems like if I'm going to treat joint dys function why not just go see Jerry in Henderson?

I have a concern regarding the physicality of his treatment - considering that I had surgery on May 9, the last thing I would want to do is to re-injure or injure the pelvic area muscles in any way. As you probably know, Dr Hibner instructs his patient s to wait at least 8 weeks before starting any kind of physical therapy, and i'm coming up on week 8 pretty soon.

Just how intense physically is Jerry's treatment?

How much does Jerry charge for his treatment?

I'm still out of the office on disability and i suppose going to nevada to see Jerry is something that if I'm going to do, would be best to move forward with while i have free time.

Thanks again for your time and info
Jerry's treatments tend to be quite gentle. I would have the same reservations you have though about getting anything done soon after surgery. If Dr. Hibner said PT can be started at 8 weeks I would think Jerry could work on you at 8 weeks, because his work is really less rough than most PT. He believes in NEVER causing pain, his philosophy is, "if it hurts, don't do it", whereas if you have done any PT you might have experienced that many do encourage continuation of excercises or body work which causes pain. Still, I think you should ask Dr. Hibner specifically how long he recommends you wait before PT Manual Therapy - he should have an answer for you as his PT Loretta does Manual Therapy (but not Hesch Method). Also ask Jerry how long out from surgery he would be comfortable working on you, I expect he will be conservative about it.
Jerry charges $100 per hour. Average 3 days of work: 1st day 2.5 hours, second day 1.5 hours, 3rd day 1 hour, so approximately $500 total IF he can help you. That's a generic estimate, individuals can need more or less time. The nice thing is, if he can't help you he will know this very quickly and you will be out 100 bucks plus travel. He does not have a generic program, he checks your joints each individually and if he can't find an abnormality he will tell you that you are not a candidate for his work and you will be outta there. Sounds like that would not happen though as you have been advised consistently that your pelvis does need to be balanced.
pelvic pain started 1985 age 14 interstitial cystitis. Refused medical care from age 17, did GREAT with self care for years.
2004 PN started gradually, disabled by 2009. Underlying cause SIJD & Tarlov cysts
improved with PT & meds: neurontin, valium, nortriptyline, propanolol. (off nortriptyline & propanolol now, yay!)
Tarlov cyst surgery with Dr. Frank Feigenbaum March 20, 2012.
Results have been excellent so far; but I won't know my final functional level for a couple of years.

Pelvic misalignment, joint dysfunction, chronic ligament pain all played a huge role in PNE for me and I've known many people over the years who've had PNE related to joint/ligament problems, pelvic misalignment, and pelvic instability.
PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

Onset PN/PFD/centralized pain in Oct 06 after years of athletics, nursing career and dog training. PT for two years with improvement, now go for tune-ups and pain management. Stopped Cymbalta, was on M.S. Contin, then Kadian, and briefly Methadone for pain management, now off those meds and pain is well managed with Buprenorphine. Followed my pain management specialist.

Hi,
I recently found a Doctor with whom I am greatly impressed--Dr. Daniel Kirages (DPT, OCS, FAAOMPT) who teaches and works at USC right in L.A.. He is the ONLY PT that specializes in the MALE PELVIC FLOOR in S.CAl. that I have found. I just began PT with Dr. Kirages and I have found him to be Very Informative and Thorough. I am Very Impressed with his knowledge and technique which is very comprehensive-- incorporating massage, Trigger Point Release(internal/external), stretching, and adjustments. Additionally, he and his office staff create a very relaxed and comfortable atmosphere and he puts you at easy during the exam and therapy. It was, also, helpful to have the opportunity to speak with him on the phone prior to my examination and reviewed my case. Feel free to contact me with any questions. (USC, also, takes most insurance plans.)
I wish you well,
Shon

Shon, I'm glad to hear you have found someone you are comfortable with. Do you know if he has much experience in treating patients with pudendal neuralgia? Please keep us posted on how things go for you. ;)
