Hi Merrie

I also had varices show up in my Potter MRI around the perineal branches.

That was her most significant finding.

Does Dr Hibner propose doing his usual TG surgery of removing ligaments, freeing up alcocks etc or does he have a more scaled down version that focuses on just that location?

Hi Merrie,
The only problem with Dr. Dellon is that he doesn't take insurance and demands cash upfront.It is a huge problem from a patient's prespective.if you don't have much sitting problem then dorsal and perineal nerves are your issues.

Did you actually talk to Dr. Potter because sometimes the MRI department can mislead you.Please email Dr. Potter directly to clarify things.

Hi AJIrish,
He does a modified version of Robert's traditional TG approach.He doesn't sever the ligaments like Robert or the Houston team does,but cuts 1/3 rd of the Sacrotuberous ligament and then repairs it with a grafted ligament.However,he does sever the Sacro Spinous ligament.
As far as the dorsal nerve is concerned,he is ready for the dorsal nerve decompression surgery.The perineal branch is very small.It is about 1 cm long and is a extention of the dorsal nerve.

So If I follow Dr Hibner now has two surgical courses of attack given the patients symptoms and MRI...

1) Fairly Invasive 'Modified TG surgery' for ligamets and alcocks canal (Originated by Robert)

or

2) Less Invasive 'Dorsal Surgery' for Dorsal and Perineal Nerves (Originated by Dellon and Aszmann)

Ali-

I do have horrendous sitting problems, infact I have horrendous pain through the whole PN distribution on my right side. I can feel the enrtire nerve on fire from my sacrum all through my pelvis, vaginal wall, clitoral area 24/7. I also have rsd pain throuhgout my whole right hip, right buttock, entire scarum and right inner thigh. SO I fear I have multiple locations where the PN is injured and/or entrapped. Thus my fear about what to do next.

Hi Paulette,
I will pm you her email address as well her secretaries email.The tech did the same thing to me regarding the Pudendal Artery Test and the Penile Suspensory ligament and I made the trip to New York.Dr. Potter got really mad with her staff and the techs .
