Dr Hibner detaches both the SS and ST ligaments, but then reattaches the ST ligament (after working on the nerve) for added stability. I notice nothing regarding my SS ligament being detached.

This is explained nicely on his website.

Make SURE your surgeon plans on descarring your nerve. I had a classic entrapment between the sacrospinous and sacrotuberous ligaments. The majority of the surgery, time wise, was spent removing scar tissue.

Before my surgery, I took compounded Ketamine, Baclofen, and Lidocaine suppositories, and they didn’t do a thing for the pain.
