I just want to add that Clear Passage was covered by my health insurance - Florida Blue Cross and Blue Shield. Clear Passage does provide you with the statements to submit yourself. They are a small clinic and that is not uncommon for small PT clinics. They charge the exact same rates that I was pay...

Hi Jax, That is great that you found a combo that worked for you. I was started on Cymbalta last month but it made my heart race, so had to stop it after a week.
