I had the same experience with gyns who do not specialize in pelvic pain and the doctor I see at this time related to my pelvic pain is also a urologist. Itching can be a symptom of nerve dysfunction. I occasionally have it with PN. In addition to PN I have had had obturator neuralgia off and on over the years (the nerve innervates a small spot on the inner thigh) and in that case, itching is my only symptom.

I have also thought there could be an underlying issue affecting the pudendal nerve that is asymptomatic but coming off an antidepressant makes it symptomatic.

It sounds like your symptoms are in the distribution of the pudendal nerve. In my opinion, PT is worth a good try but if you find it isn’t helping after a couple of months or your symptoms are increasing from the PT I think it is worth reconsidering with your doctor. Marky, From what I have gathered from PTs, relief should be significant in the first three months of therapy. In my experience, PN can cause the pain you describe. I suppose it could also be muscular. If your pain is primarily muscular, I think you will see at least some improvement through the PT within a few weeks.
